I Have Forgotten My User Code

Step 1

To retrieve your user code, enter the email address associated with your clicSÉQUR account. If your account type allows it, you will see a message indicating that your user code has been sent to you by email.

However, for security reasons, it may not be possible to send you your user code by email. If this is the case, you will automatically go to the next step. The way you will receive your code will depend on how your identity was verified when your account was created. (with your health insurance number or your social insurance number).

Step 2

Identity Verified with Health Insurance Number

If you are asked to provide your medical insurance number when retrieving your user code, you must first contact the Régie de l’assurance maladie du Québec to get a new activation key. This activation key is required to complete the retrieval process for your user code. For security and confidentiality purposes, your activation key will be sent by mail, just as it was when you first created your account.

  • Wednesday: 10 a.m. to 4 :30 p.m.

Once you have received your activation key, you may restart the process to access the service you would like to use. You will have to provide information about your health insurance card (including your health insurance number) and the activation key you received from the Régie. Once you have done so, you will go on to the next step.

Identity Verified with Social Insurance Number 

If you are asked to provide your social insurance number when retrieving your user code, simply provide the required personal information (including your notice of assessment number). Once you have done so, you will go on to the next step.

Step 3

At this step you must:

  • click on I agree;
  • confirm the accuracy of the information provided above by selecting Yes;
  • click on Continue.

The user code associated with your clicSÉQUR account will appear. Click on Continue. If more than one user code appears because you have more than one account, select the user code you wish to retrieve and click on Continue

This will take you to the next step. 

Step 4

You will have to change your security questions and answers. For this purpose

  • choose four new security questions;
  • enter the corresponding answers;
  • click on Continue;
  • confirm your choice of security questions and answers by entering your password.

Note that you can also create a new account. However, you will have to re-register for the online services of participating departments and agencies.
